Yeah, this is a common problem, getting physicians to prescribe the correct dose of opiate. Physician’s nortoriously underdose, which is medically incorrect. You have to slightly OVERdose in the beginning to get ahead of the pain, as though it’s a wildfire, and then rachet it down a bit. In the long run, less medication is needed for a continuous maintenance dose that way than gradually building up, which never really puts the fire out, so to say. Certainly, those first few days on a large dose of opiate is going to knock you for a loop. But, a huge part of that “gorked” period is actually due to finally being comfortable enough to get deeper stages of sleep, which can’t be obtained when you are in pain. Those deeper stages of sleep accumulate into a “sleep debt” that, when you finally achieve sufficient pain relief, you sleep like the dead for about two to three days to catch up on the debt. But, just as soon as that sleep debt is paid off and you have a few pain-free days and adjust the med as necessary after that, you’ll be just as clear-headed as anyone else.
